Records Review Consultation — $250
Not every veteran needs a Nexus Letter or a Comprehensive Independent Medical Examination (IME). A Records Review Consultation provides a professional review of your VA claim to help determine what evidence may strengthen your case before investing in a more comprehensive evaluation.

Identification of strengths and weaknesses within your claim

Nexus Letter — $599
A Nexus Letter is an independent medical opinion (not an IME- see below) that addresses whether a veteran’s mental health condition is at least as likely as not (50% probability or greater) related to military service or to another service-connected disability. Our opinions are evidence-based, individualized, and supported by a thorough review of the available records, current scientific literature when appropriate, and a comprehensive clinical interview.
This evaluation includes:
Review of relevant military, VA, and civilian medical records
60–90 minute clinical interview
Mental Status Examination
Standardized symptom measures as clinically appropriate, including PHQ-9 (Depression), GAD-7 (Anxiety), and PCL-5 (PTSD Checklist for DSM-5)
Independent Medical Opinion utilizing the VA’s “At Least As Likely As Not” legal standard
Detailed written Nexus Letter with supporting clinical rationale
Best for veterans who:

Comprehensive Independent Medical Examination (IME) — Starting at $1,995
An Independent Medical Examination (IME) is a comprehensive forensic psychological evaluation designed for veterans pursuing VA disability claims, appeals, or challenging unfavorable Compensation & Pension (C&P) examinations. Unlike a Nexus Letter alone, an IME provides an in-depth evaluation of diagnosis, symptom severity, functional impairment, and the relationship between your condition and military service.
Your IME may include:
Comprehensive Record Review — including Military Service Records, VA Medical Records, Civilian Medical Records, Prior C&P Examinations, Rating Decisions, and Supporting Documentation.
Clinical Evaluation — including a 2–3 hour clinical interview, Mental Status Examination, DSM-5 diagnostic assessment, and occupational and social functioning assessment.
Standardized Assessment Measures — when clinically indicated, may include PHQ-9 (Depression), GAD-7 (Generalized Anxiety), PCL-5 (PTSD Checklist for DSM-5), CAPS-5 (Clinician-Administered PTSD Scale for DSM-5), MCMI-IV (Millon Clinical Multiaxial Inventory-IV), and additional psychological testing as clinically indicated.
Comprehensive Documentation — a detailed forensic psychological report, an Independent Medical Opinion (Nexus) when appropriate, a VA Mental Disorders Disability Benefits Questionnaire (DBQ) when clinically appropriate, and evidence-based rationale supporting all diagnostic and forensic opinions.

A Nexus Letter is a written opinion from a qualified clinician that addresses whether a current medical or mental health condition is “at least as likely as not” connected to events, injuries, or exposures that occurred during military service. The VA often requires this kind of medical nexus opinion to establish service connection, especially for conditions like PTSD, TBI, depression, anxiety, and ADHD-related symptoms that surface or worsen after discharge. A well-documented, clinically sound Nexus Letter gives your VA claim the evidentiary support it needs.
How a Nexus Letter Can Help Your VA Claim
Many veterans have their claims delayed or denied simply because the VA does not have a clear, clinician-documented link between their diagnosis and their time in service. A thorough Nexus Letter can strengthen your claim by clearly documenting your current symptoms, referencing your service history and any in-service stressors or injuries, and offering a reasoned clinical opinion on the likelihood of a service connection. Every Nexus Letter I prepare is grounded in a comprehensive clinical evaluation, not a brief phone call or a form letter. My evaluation includes:
A 60–90 minute Statement of Origin (SOF) clinical interview and diagnostic evaluation
A clinical personality assessment, the Millon Clinical Multiaxial Inventory-IV (MCMI-IV)
The Clinician-Administered PTSD Scale for DSM-5 (CAPS-5)
Following your evaluation, you receive a complete, individualized Nexus Letter that documents your diagnoses, summarizes the clinical findings from your assessments, and provides a clear, professional opinion on the likelihood that your current symptoms are connected to your period of military service.
Nexus Letter Rate
The complete Nexus Letter service, including the SOF clinical interview, MCMI-IV, CAPS-5 assessment, and your final written Nexus Letter, is $599.00 total. There are no hidden fees or add-on charges.
Ongoing Care: Neurofeedback for Trauma, PTSD & ADHD
In addition to Nexus Letter evaluations, I am a community care provider offering neurofeedback therapy to veterans and community members throughout the Upstate. Neurofeedback is a non-invasive, evidence-informed approach that helps retrain the brain’s self-regulation patterns and is often used alongside traditional treatment for trauma, PTSD, and ADHD. If you are already service-connected or are working through the effects of your service, neurofeedback can be part of your long-term care plan.
